I added the version number as it differs from other versions I've submitted.
I removed (PQII DISK 2) and (PQII DISK 3) as it seems redundant.
I added [cp manual].

Also resource.cfg could be removed from the first ima file as it is not shipped with the original disks (it's the users settings when installed - modified dates can confirm this).

Good work, your suggestions seem logical to me, it is great to have the version info and the more info flag about the cp (cp = copy protection, right?) manual is ok if present, i've no knowledge about that :P
As for the media labels, i guess they were present in the original disks, probably during install or play the game will ask for PQII DISK 2 and 3 but as you said they are kinda redundant since it is obvious which one is the disk 2 and 3 with those labels. I guess both options are acceptable and will let other renamers comment which one they find better.

About your 3rd point, i though the original media was (always?) write protected so the original disk content couldn't be altered. I guess i was wrong...
You're right about it, the objective (in this case) is to have an image 100% equal to the original media, i suppose it is playable without the cfg (that it gets generated at first run), so the file shouldn't be there indeed. In fact, this is one of the reasons there are so many alts [a], many of these sets would change each time they were run (configs, hi scores, whatever would get updated) and dumping them will generate tons of different images/hashs :)

Unfortunately the original media is not always write protected; there's a switch on the disk that can be manually changed. Usually, if these games are played on a PC without a hard drive, the original disks were  usually the place game states were saved.

In addition the save game states from both images should be removed (ie. Disk 1: kq3sg.1 kq3sg.2 kq3sg.3 kq3sg.4 kq3sg.5, Disk 2: kq3sg.1).
Alternatively I have uploaded some images I have dumped to the ftp.

Alternatively I have uploaded a v0.000.051 to the ftp.
