Doom Editor Utilities v5.21 (1994)(ID Software) from IBM PC Compatibles - Games - Addons & Patches - [IMG] should actually be Lost Episodes of Doom, The (1995)(Sybex). I created an image of my floppy for the Lost Episodes of Doom with both RawRead and WinImage, the resulting hashes were the same for both tools and match Doom Editor Utilities. DEU is included on the disk, but JPTR_V40.EXE in the image is the so called Lost Episode. I've attached a picture of my floppy disk to verify my statement.

Edit: I checked the Doom Wiki's entry on the Lost Episodes of Doom and it lists the SHA-1 hash as:
e820373ae6bd18282bec8f3aa5b40c71a6105637

Edit 2: I should note that the other entry for Lost Episodes has all the contents, but the person who created it didn't image the disk. Rather, they created an image in WinImage and then injected the files into it. You can tell this is what happened since, if you view the file in a hex editor, it says "Disk formatted with WinImage 6.50". This appears in any blank image you create with WinImage (rather than performing a raw read of the floppy).
